Clinical biophysics investigates the relationship between non-ionizing physical energy and the human body. Although several types of electrical stimulation devices have received US FDA approval for orthopaedic application, the use of Pulsed Electromagnetic Field (PEMFs) play a central role in joint biophysics. This narrative review aims to summarize the current evidences on the efficacy of PEMF-therapy in the treatment of knee articular diseases. Preclinical studies have assessed the effects of PEMFs on chondrocytes, synoviocytes, articular cartilage explants and animal models, showing positive effects of PEMF-therapy on cells proliferation, extracellular matrix (ECM) production, chondrocytes apoptosis and inflammatory cytokines down-regulation. Currently, PEMF-therapy is a valid option in the conservative management of several knee articular diseases, including early OA, patellofemoral pain syndrome and SONK. PEMFs could be also used as an adjunct after an arthroscopic knee procedure or TKA implantation, in order to control the joint post-operative inflammatory state.
AFTER MRI INDIVIDUAL A: had severe femoral avascular necrosis. He suffered from pain and had difficulty walking. Both femurs were already in such a poor shape, so that hip replacements would have been very problematic if possible at all.

Dr. Love's comments on this study. First enhancing performance with athlete's using PEMF is not novel. The Olympic Russian athletes have been optimizing their performance for decades using PEMF. Although it is refreshing to see studies using PEMF to enhance performance being published in North American and thus will hopefully increase PEMF's awareness here in Canada on its efficacy to impact athletic performance.
PEMF is not considered doping because no chemicals are administered to the athlete. In this study a small arm PEMF device was used. Although they found positive results, using a whole body system to stimulate the production of ATP will stimulate the whole body and it is reasonable to state this would have a more advantageous effect.
As elite athletes and performers expend a great deal of energy practicing, performing and competing, PEMF in Dr. Love's opinion should be used consistently to maintain optimal and peak capacity. PEMF acts on the transmembrane potential. Red blood cells (RBC) function to carry oxygen. If their charge is decreased they stick together. This is called rouleaux. PEMF therapy was associated with a reduction in joint pain in those with OA compared with placebo. In conclusion, this review suggests that PEMF therapy, compared with placebo, has clinically significant effects on joint pain in individuals with OA. Further, we observed a statistically significant beneficial effect of PEMF therapy on joint stiffness and physical function.
Dr. Love's personal study: 87 yr. old male, history of two TIA's. This man is Dr. Love's father and was treated by an associate she referred her father too. She felt he was not a candidate for a total knee replacement due to his history of two strokes. His knees had significant osteoarthritis in both knees. He was booked to have knee replacement surgery and Dr. Love begged her father to try PEMF. He went for five PEMF treatments, over a course of a couple of weeks and his knee pain disappeared, his activities of daily life improved and he cancelled his knee replacement surgery. To date, he goes for maintenance PEMF treatments in his city every three weeks and remains a very active man who looks after his 1/3 acre yard by himself and walks his Calahoula hound called Olive almost every day, weather permitting.
The consequences of these TBI’s of been brought to the forefront recently with sports concussions. New approaches to managing this important condition are necessary. Modern evidence now suggests that even though somebody has recovered from their concussion, there are residual long-term effects in the brain.
Other evidence indicates that the use of pulsed electromagnetic fields, of various kinds, early in the injury process helps to decrease one of the major aspects of the injury, which is inflammation in the brain. In searching for mechanisms, it was hypothesized that myocyte protection may be due to a PEMF-related opening of adenosine-triphosphate-activated potassium channels (KATP) (Barzelai, Dayan et al. 2009, Fixler, Yitzhaki et al. 2012, Aharonovich, Scheinowitz et al. 2016). These channels are present in myocyte sarcolemma, but most are normally not open (Nichols, Singh et al. 2013). However, reduced blood flow can open these channels causing increased outward K+ flux. This produces a cellular hyperpolarization and a more rapid AP repolarization. This in turn reduces the action potential duration (APD) and also reduces Ca++ entry into the myocyte thereby reducing myocardial contractility. This would conceptually provide a temporary energy conserving process and, if augmented by the application of the PEMF, could help explain the findings.
